Forms

Counseling can begin after the 3 required forms below are received:

1. Client Information Form
2. BPS Form
3. Faith Consent Form

Please print, complete in full, and email or bring to your first appointment, along with a photocopy of both the front and back of your insurance card. Signatures can be done in person at the first appointment.

If completed forms are not received prior to the appointment, they will be filled out during the initial session.
